These instructions are color-coded so you know exactly what information must be completed by whom:

Red: Applicant completes
Green: Your notary completes
Blue: Leave this section blank

Note: To print these instructions with the color-coded comments, select "Print: Document and comments." Otherwise Adobe® will simply print the blank application.

Step 2: Complete the application

Open the application set (three pages in Adobe® *.pdf format) by clicking on the forms box below:

(page 3)

The application is preformatted to allow you to complete the form from your browser window.

When the application has opened, click your mouse cursor in the "Name" field on page one (#1), then enter your name as you want it to appear on your notary commission.  Move from field to field in the application by pressing the 'TAB' key on your keyboard to move forward, or 'SHIFT' + 'TAB' to move backward.

The form will automatically place your information in related fields as you type (e.g., when you type your name on page 1, it is also added to the "Applicant" and "Principal" fields on page 2 and show you exactly where you need to sign!)

When you have completed the application, select the 'PRINT' command. Note: To print the application with your information, select "Print: Document and comments," Otherwise Acrobat® may simply print the blank application.

  1. Copy Page 2 onto the back of Page 1. The two (2) page application must appear on the front and back of a single piece of paperThis is critical as submissions appearing on two separate pages will not be accepted by the State of Indiana.

  2. Complete the application. Make certain that you sign as the "applicant" in Section 9, and as the "principal" in Section 10. Refer to the instructions if you have any questions.

  3. Have your signatures notarized in Section 9 and Section 11. Make certain the notary has placed his/her seal in Section 9 and 11 as well.

Step 3: Submit the application

  1. Recheck your application for accuracy.  Incomplete applications will be returned by the Secretary of State's office. Complete the Application Checklist (page 3).
